Capital Medical Group is seeking an APP to join our inpatient clinical team. The APP will provide high-quality, patient-centered care to hospitalized patients while working collaboratively with physicians and an interdisciplinary healthcare team.

 

The APP will evaluate and manage patients throughout their hospital stay, participate in daily rounds, develop and implement treatment plans, coordinate care, and facilitate safe and timely transitions of care. The position requires strong clinical judgment, effective communication, and the ability to prioritize and manage multiple patients in a fast-paced hospital environment.

Essential Duties and Responsibilities

  • Perform comprehensive patient assessments, histories, and physical examinations.
  • Evaluate and manage hospitalized patients in collaboration with physicians and the interdisciplinary care team.
  • Develop, implement, and modify treatment plans based on clinical findings and patient needs.
  • Order and interpret diagnostic tests, laboratory studies, medications, and treatments within scope of practice.
  • Participate in daily patient rounds and multidisciplinary care planning.
  • Recognize changes in patient condition and initiate appropriate interventions and escalation of care.
  • Perform procedures and clinical interventions consistent with training, competency, credentialing, and organizational policy.
  • Coordinate consultations and follow-up care with physicians and other healthcare professionals.
  • Facilitate admissions, transfers, discharge planning, and transitions of care.
  • Provide education and support to patients and families regarding diagnoses, treatment plans, medications, and follow-up care.
  • Maintain accurate and timely clinical documentation and complete required billing and charge-capture activities.
  • Participate in quality-improvement, patient-safety, educational, and professional-development activities.

Required Qualifications

  • Graduate degree from an accredited Nurse Practitioner or Physician Assistant program.
  • Current national certification appropriate to the provider's profession.
  • Current or eligible licensure in the applicable jurisdiction.
  • Appropriate prescriptive authority and controlled-substance registration, as required.
  • Current or eligible DEA registration, as applicable.
  • Ability to meet all organizational employment, occupational health, background-screening, and other applicable requirements.
